Dave Winer thinks RSS aggregators need a lot of work. He says:
Now, the problem is, that while Yahoo’s interface is pretty good, and lots better than most of the rest, it still shows you old items before new ones. Any of the competitors could leapfrog Yahoo. Want to give it a try? I can be hired as a consultant. Seriously. Let’s get going on this.
I’d be ready to be dazzled. For now, I’m Catching a 6 a.m. flight from LaGuardia (if I’m lucky). Blogging on Friday will hinge upon Wi-Fi in the Midwest.
I don't mind seeing items more than once because it gives me the ability to revisit recent items more recently. What Yahoo and Sage miss (the two aggregators that I use) is the comments on items. A feature of my ideal aggregator would be for me to be able to easily mark items of interest that would show when new comments have been posted.
Regarding seeing new comments posted: That's up to the feeder (the original blog or whatever is providing content) to update the feed content, not the aggregator. It's the same old principle of Garbage In Garbage Out.
